Description: The Medicare Rural Hospital Flexibility Program (Flex Program), created by Congress in 1997, allows small hospitals to be licensed as Critical Access Hospitals (CAHs) and offers grants to States to help implement initiatives to strengthen the rural health care infrastructure.

Description: The Small Rural Hospital Improvement Grant Program (SHIP) helps rural hospitals of 49 beds or less do any or all of the following: 1. Purchase equipment and/or training to attain value-based purchasing provision in the Patient Protection and Affordable Care Act 2. Join or become accountable care organizations, or create shared savings programs per the ACA 3. Purchase health information technology, equipment, and/or training to comply with meaningful use, ICD-10 standards, and payment bundling(ACA)

Description: The SORH program was developed in 1991 as a Federal-State partnership. It features a single grantee from each State and requires a State match of $3 for each $1 in Federal funding and has leveraged in excess of $250 million in State matching funds. Currently, 37 Offices are located in State health departments, 10 in academic settings and 3 in non-profit organizations.

Description: Disabled Children's Relief Fund (DCRF), a non-profit 501©(3) organization, provides disabled children with assistance to obtain wheelchairs, orthopedic braces, walkers, lifts, hearing aids, eyeglasses, medical equipment, physical therapy, and surgery. Blind, Deaf, Amputees, and children with Cerebral Palsy, Muscular Dystrophy, Spastic Quadriplegia, Encephalitis, Rheumatoid Arthritis, Spina Bifida, Down's Syndrome, and other disabilities receive assistance.

Description: In a short three years, the Foundation has awarded seventy-seven grants to individuals with special needs and the organizations that support them totaling over $60,000.00. The Foundation has approved grants for services and equipment such as; physical therapy, occupational therapy, speech therapy, medical equipment, a toilet chair, a wheelchair, a wheelchair lift, summer camp and a tandem bike which was featured by Team Myles in the 2011 Cleveland Triathlon.

This program does not provide funding for ABA or other types of therapy, camp tuition, respite care, fencing, trampolines, swingsets, trips to Disney World, etc. Do not apply if your annual net income exceeds $50,000.
